Accessing your GP health record

The easiest way to access your GP health record is using the NHS Wales App.

To access your GP health record online, you must be:

  • registered with the surgery
  • aged 16 or over
Getting help with your GP health record

If you only have access to your summary record, you’ll see a message on your GP health record. The message will let you know that you can request access to more information.

To see more information in your record, such as test results and immunisations, you can contact us and request access to your “detailed coded record”.

You can also contact the surgery for help if:

  • you need a printed copy of your record or to be sent a digital version
  • you’re unable to see your record
  • information is missing or has disappeared from your record
  • there’s incorrect information in your record
  • you can see information that should not be there (for example, you can see information that’s not yours)
  • you do not want to be able to see your record
  • someone is pressuring you for information in your record

What’s in your GP health record

Your GP health record may include information such as:

  • allergies
  • immunisations (vaccinations)
  • health conditions
  • test results such as blood tests
  • consultations and events such as appointments
  • your personal information (such as date of birth)
  • allergies and adverse reactions
  • medicines you have been prescribed (both in the past and ongoing)

Your GP health record does not include information kept by other services that has not been sent to the surgery. This includes services like hospitals, dentists and opticians. To access this information, you will need to contact the service directly.
